Clinic Procedures

Medications

Administration of prescription and over-the-counter medications during school hours is discouraged.  Medications prescribed three times daily often can be given before school, after school, and at bedtime. However, if administration of the medication is required, the parent must complete the Administration of Medication Request Form.  Students may not have prescription and/or over -the-counter medications in their possession.  ALL medications must be kept in the clinic.

Health Management Plans

The parent/guardian will need to complete a health management plan for students with medical conditions that may require assistance and/or monitoring during school hours (i.e. asthma, severe allergies, diabetes, etc.).  Asthma and allergy health management plans can be printed below.  All other forms will need to be picked up in the clinic.
